　　An Oklahoma City waitress died on March 4 after falling down a restaurant’s dumbwaiter shaft. Investigators believe the 21 year old was loading or unloading the dumbwaiter when it suffered a mechanical malfunction, fell to the ground floor and pulled her with it. A full investigation into the accident is underway.
